INSTRUCTIONS: Updates: Complete the Callsign, email and name items plus whatever has changed, click Submit button. New Applications: Complete all fields, click the Submit button then complete the PayPal screen that is presented. If you do not wish to use PayPal, download the paper copy and send or bring it in.

New Applications: Complete all fields, click the Mail Form button at the bottom, then complete the PayPal payment form that will be displayed.

To submit changes, simply complete the ID information and complete any items that have changed.
